OUR SERVICES

Advanced Geriatric Care and Family Practice Associates specializes in Family & Geriatric Medicine.  We are a primary care office and are affiliated with Monarch Healthcare HMO plans, most PPO & Medicare.  We see patients from age 12 and older.  

OFFICE APPOINTMENTS

We currently have 1 office location in the Taj Mahal Medical Center in Laguna Hills, Ca. 

Our office is open Monday - Friday from 8am - 5pm.  Appointments can be made by calling 949-305-2660. 

Our providers rotate clinic hours so they can also provide services to our home bound patients in the community. Patients may schedule an appointment with any provider for office visits.

 HOME VISITS & ASSISTED LIVING VISITS & BOARD AND CARES & SKILLED NURSING FACILITY VISITS

All of our providers visit patients in facilities, board and cares and in their homes throughout south Orange County.  We are pleased to provide this service for those patients that are very limited in their ability to come to clinic office appointments.  Visit frequency is based on the individual needs of each patient.  

Home visits are available in a limited geographic area; please ask our office for more details.
