Advanced Geriatric Care & Family Practice Associates specializes in Family Practice and Geriatric Medicine.  We see patients age 12 and older.  We are a provider for Medicare, most PPO's and Monarch Healthcare HMO plans.

Founded in August 2005 by Dr. Roger Tran, MD, Advanced Geriatric Care & Family Practice Associates, Inc. strives to provide personalized, high quality medical care to their adult and senior patients in South   Orange County.  
Dr. Tran enjoys the fact that patients are able to establish care with him and he is able to follow their medical care from early adulthood through their senior years. 

Services include:  Preventive Care, Complete Adult Medicine, General Family Practice (ages 12 and older) and Comprehensive Geriatric Medical services.  We offer Memory Disorder Assessment, Chronic Disease Management, Nursing Home Care and home visits for those patients that are home bound. 

Advanced Geriatric Care & Family Practice Associates accepts Medicare, Medi-Cal, most HMO, PPO, & private insurances. We happily welcome new patients. 

Office Hours are Monday - Friday 9:00AM-5:00PM. We close daily from 12pm-2pm.
